You'll work alongside other healthcare professionals to develop and implement care plans that support patients' well-being and recovery.
To qualify, you must hold a Master's degree in Social Work and have a valid Medical Social Worker license in the state.
Experience in home health or hospice is preferred, but not required.

Maintains clinical records on all patients referred to social work.
Provides information and referral services for Organization patients and families/caregivers regarding practical and environmental needs.
Qualifications Graduate of a Master's of Social Work Minimum of one year's experience in health care.
Experience in a home health care preferred.
